An all-in-one care coordination platform

Hypercare is a mobile-first secure clinical communication and collaboration platform. It allows clinicians to share patient information and files securely, and integrates work schedules to ensure that the users can connect to the right provider at the right time. Hypercare has partnered with Southlake @ Home to help coordinate complex and multidisciplinary medical care for patients transitioning from the hospital back into the community, and collecting data on workflow process efficiency and user satisfaction. As medical care around the world is moving towards community-based and team-based patient care, Hypercare hopes to help coordinate a currently fragmented process into a seamless experience for patients and providers.
